Then you will never find the root cause nor are you able to inform
developers or users about mistakes.

This happen if JOSM fails to upload correctly and the user does not
update data before trying to upload again. In fact there is even a bug
in JOSM where the upload finishes but JOSM never gets that important
answer from the server.

Do not know about other editors.
