Allan, thank you for the feedback on some metrics, which inspired me make a modest proposal:
Since question D5 mentions that "no exact location is required", perhaps a drop-down field to select only the country instead of asking for an exact location on the map could improve acceptance.
I can offer a question "Which country do you currently live in?" with this drop-down list. The question and country list has already been translated into the following languages: ar, bn, bs, de, el, en (base language), es, fa, fi, fr, hi, hu, id, it, ja, ko, mr, nl, pa, pl, pt, pt-BR, ro, ru, sv, swh, ta, te, tr, uk, ur, vi, yo, zh-Hans (thanks to Translators without Borders).

Another thought regarding the volume of participation: the survey is anonymous and therefore no participant data is linked to the responses. Is there a restriction that prevents the authors from creating a table of participants with the email addresses of all OSM users and sending out invitations to participate?
E.g. question D3 "Optional Demographic: How long have you been involved in the OSM project?" could thus be pre-filled based on the registration date.
