[Tile-serving] [mod_tile] Add copyright and permission info. (#6)

Would it be acceptable to merge this pull request initially, which adds the copyright template but uses the non specific "(c) 2013 mod_tile contributors". And then in a second wave add the proper author names and dates from the revision history file by file, starting with the most important files?

The non specific author information with the license template seems no worse than what we have now, and perhaps will remind / encourage people to add the correct author information when they commit new code.
