[Tile-serving] [osm2pgsql] planet_osm_ways coming out bloated (#111)

I'm wondering if deferring the `btree (id) WHERE pending` index as proposed in #105 would allow the use of [HOT tuples](http://git.postgresql.org/gitweb/?p=postgresql.git;a=blob;f=src/backend/access/heap/README.HOT;hb=HEAD)
