[Tile-serving] [osm2pgsql] Memory leaks of C++ version (Windows-only?) (#228)

#237 and #240 should have fixed most of the leaks. There are a couple of one-time allocations in `set_prefix_and_tbls()`, buffers in middle-pgsql.cpp and the last round of keyvals in parse-pbf.cpp that remain. None of them is critical.

@alex85k could you check that there are no other significant leaks reported for the windows built?
