[Tile-serving] [osm2pgsql] Make more of osm2pgsql threading aware (#110)

so, I can confirm that 9030e137a892fad16e1d19f3b85142faaadb72ab fixes the issue - but I don't have any speed remarks because it requires additional indexes which I didn't create until partway through the run.

I suppose there's two questions

* What is the speed like if all the indexes are present
* Are the changes in output without 9030e137a892fad16e1d19f3b85142faaadb72ab acceptable changes?
