[Tile-serving] [osm2pgsql] Good cheaper indexing (#171)

I have used "not null" for my own indexes after import. What indexes are
created on the fields other than the Geometry and Primary Key by Osm2PGSql

BTW, If you want to keep the Nulls for any reason to test for them
occasionally you can use NULLS LAST to put them at the end and keep the
speed for the normal lookups.
