[Tile-serving] [osm2pgsql] Good cheaper indexing (#171)

Actually I partially botched a database import and tried to rebuild some indexes from scratch, ended up having the issue above. Mapnik rendering is very slow without indexes (I don't know much about it and followed switch2osm), so having some default indexes in osm2pgsql would be nice to have, at least for beginners.
