[Tile-serving] [mod_tile] mapnik-config parameters don't work with 2.0 (#84)

Drop support for mapnik 2.0 by checking `mapnik-config --version-number > 200000`. 2.0.0 is > 3 years old at this point. Alternatively just call `mapnik-config --cflags` which has been around since the beginning and is a complete dump of required flags.
