[Tile-serving] [osm2pgsql] Make more of osm2pgsql threading aware (#110)

There's stuff from here we want to keep, but given that it has an intermittent core dump problem that takes 2-5 imports to reproduce and that we've had absolutely no luck finding the cause of, I'm going to close this PR.

The general view has been to do a targeted rewrite on top of #187, as C++ should make some of this threading easier, so no work has been done lately on finding the problem in this branch.
