[Tile-serving] [osm2pgsql] Conversion to C++ (#187)

> @pnorman OK, I'll add some small Windows-dependent PR-s to Mapbox branch after finishing the test environment setup (will make sure they work on MSVC, Mingw64, Ubuntu 14.04 and FreeBSD10). Will discuss in corresponding commit comments and PRs.

`make check` does pass with c615097a1e0bbd16cd5505143a922b69f47e602e on Ubuntu, if you make an empty io.h file to fix that bug
