[Tile-serving] [osm2pgsql] Relations using way (#372)

After IRC discussion, the way forward with #213 is to error on append without slim.

If someone wants to implement `middle_ram_t::relations_using_way` they can do a pull request, but otherwise this closes #213 as wontfix.
