[Tile-serving] [osm2pgsql] Add cmake build scripts and appveyor.yml (#460)

We should be specifying it for all builds, including non-travis. With autotools we're using `AX_CFLAGS_WARN_ALL` and `AX_CXXFLAGS_WARN_ALL`

`-D_LARGEFILE_SOURCE`

node-presistent-cache.cpp has `#define _LARGEFILE64_SOURCE     /* See feature_test_macrors(7) */`. I don't know what it does, so I'm not sure if it's best to globally define it.
