[Tile-serving] [osm2pgsql] Add cmake build scripts and appveyor.yml (#460)

The building section of the readme has about doubled in length, going from 57 to 98 lines. `README.md` is an overview. If we need in-depth instructions we can have them separate in their own file, but it looks like most of the options should be able to be explained by giving an appropriate cmake help command or pointing to a cmake page.
