[Tile-serving] [osm2pgsql] Refactor table_t constructor (#438)

I'm thinking the table class and options classes need refactoring, so that slim, pgsql output, and gazetteer output tables all can inherit from a base class, and that most of the options work would fall out of that. Think it's worth drafting up a design for table_t and associated derived classes which I could then implement?
