[Tile-serving] [openstreetmap/osm2pgsql] Set application_name with PostgreSQL (#707)

This helps viewing pg_stat_activity and logging. We might want to do this by [`fallback_application_name`](https://www.postgresql.org/docs/9.0/static/libpq-connect.html#LIBPQ-CONNECT-FALLBACK-APPLICATION-NAME) to allow the user to override it.
