[Tile-serving] [openstreetmap/osm2pgsql] Let postgres resolve non conflicting index names itself (#848)

Will this change the default index names? If so, we'll have to remember to note it on release. In any case, I'm fine with it.

I'd also strongly recommend that anyone renaming tables also renames indexes, or they are likely to find themselves confused in the future when looking at index names.
