[Tile-serving] [openstreetmap/osm2pgsql] Supporting --append in non-slim mode (#1076)

Appending in non-slim mode will not be part of mainline osm2pgsql. Non-slim mode is using much less memory now and we hope to improve that even further. Also: osm2pgsql can now reliably import multiple osm files at the same time when they are all on the command line. Together these should help most people.
