Done :-). I wrote mercatorize.pl which depends on Geo::Proj4 (unfortunately there is no Debian package for it)
This is called by the preprocessor "mercator". This way the additional dependency is optional for those clients who actually want to render lowzoom tiles (I'd expect that most will not).
This preprocessor adds mlat= mlon= tags into the lines (the wording can be changed, if it's a bad one).
Now all that is left to be done, is that Etienne rewrites osmarender to actually make use of these tags.
