A little bit of progress
ActivePerl suggests the *.pl extension is automatically associated with perl.exe - that didn't happen so updated it manually so now when I double click on tah.pl the previously mention GUI opens. It still does not run from the Start Menu link though.
There is also a ppm which I couldn't run before. Got it working and installed the 5.8.8 files as indicated on web page.
In summary
The Start Menu link still doesn't work and I don't really know what to do with the GUI.
I suspect that this may be to use my PC as a processor for Osmarendering for the greater good rather than my own experimental setup.

I'm running through the process on http://wiki.openstreetmap.org/index.php/Windows@home
I've download setuptah-0_1.exe and it appears to run succesfully.
At the final stage there is a picture (possibly in Italian) showing a GUI presumable as a hint to change the language to your own.
On closing the browser window showing this picture the install GUI highlights the Close button.
I then run the app from the Start menu but all I see is a quick flicker on the screen (probably Perl trying to start in a DOS box).
After a bit of messing around I started Perl from a DOS box as below
perl "C:\Program Files\TilesAtHome\tah.pl"
This runs a GUI showing things like "Proxy Settings" - "Authentication Settings" and buttons to "update tah" and "start tah"
Not sure if this is the real GUI or a setup window - I plan to run T at H locally to create specific tiles via a modified osmarender process.
So what settings if any do I need in this window. I've tried using it but to no result (no idea what values to use) and when I close it there are many error messages on the DOS/Perl window including many complaining about the space in C:\Program Files....
Finally, looking at tah.pl the line
$ENV{PATH} = "$dirperl\\bin;$dirinkscape;$dirsvn\\bin;$dirpngcrush;$dirzip;$ENV{PATH}";
suggests that my System Properties Environment Variables may be updated or perhaps $ENV{PATH} is a local variable.
Is this an issue?
My guess is a problem with Perl. If I run the perl.exe directly all I see is a blank DOS type window - is there a command to test the Perl?
