[uMap] Q: adding custom fields / approval workflow?

I would advise not to implement approval workflow into umap. There are plenty of CMS systems that have this functionality and more. IMHO a better investement of developer resources would be to convert umap into a module for one of these CMS-es. For example, Drupal already has at least one approvals module:
