Does this take into account ISPs that force all http traffic though  
one or two 'transparent' proxies?

Virgin/NTL/Telewest for example do this - http:// 
homepage.ntlworld.com/robin.d.h.walker/cmtips/trancache.html provides  
a comprehensive list.
