Hi Martin.
Thank you for the reply.  You are right thar for my own rendering jobs I can
use extracts, but my application is a web service so I have no control over
the area requested by the user.
That is why xapi is excellent.  It is only the reliability issue that has me
thinking about my own database server.

hand) is additionally extracting features from the planet (cities,
boundaries, rivers, countries, motorways, trunk roads ...) and merge
them with the extract for rendering low zoom tiles.

As long as you are not interested in the details in other countries
(smaller cities and streets) this allows for pleasant rendering IMHO,
"simulating" global coverage.
