In the light of https://github.com/openstreetmap/chef/issues/450 , 
presumably anyone who wants to deploy that style also needs to ensure 
that index is added too?

isn't exactly clear what the steps are for

  * Someone deploying the style for the first time (I'm guessing that
    it's automatic, but it's not completely clear)
  * Someone updating from a previous version of the style to a new one.
