That is exactly the main challenge I have as well. And like you I have 
been making them all fixme=Location approximate and then putting in a 
detailed note like where the nearest settlement is, or that there were 
too many tiny settlements nearby to decide.

Basically if the location is not on the JOG map or it doesn't come in 
really close to an obvious settlement I have been reluctant to call it 
"properly placed" and put a fixme with a note on it.

For some squares there are a great deal of fixme's and for others very few.

It is almost a two edged sword how good many of the GNS nodes come in, 
usually within 50 or 100 meters of a settlement, that I get afraid to 
move one 1.5km to the nearest settlement.

There is a measurement plugin I added to my JOSM, that while not 
perfect, I have been using to put distances in my notes.
