I would also keep waterway=stream or waterway=river when underground.

It would be layer=*, not level=*. You are in layer=0 when you have the
feature on the ground. So a river goes in layer=0, and a bridge that
goes over it will be tagged as layer=1. If a road goes through a
mountain (tunnel), you will tag it tunnel=yes + layer=-1. If you have
a river/stream/canal/ditch going underground, it would be tagged as
tunnel=yes + layer=-1. And so on.

Just keep this list informed about the tagging and import process, for
feedback.
