Failure to respond, I would suggest, needs a fairly small number of
contacts but spread over a minimum period, say a week *during which the
person has made edits* and is therefore not away or disconnected.

The final warning, which is fair, gives someone a chance to back down -
leaving a  pile of junk in the system. I would hope this could then be
reverted in a more sensitive way, but it would still have to go,
probably? As an alternative, the defined number of contacts might want
to come from an 'official' address and constitute the warning in themselves.
