Answering my own question a bit -

I found one of these with 3 segments joining at one node - but only 1 segment 
actually attached to the node! The other two appear to not have a node at the 
join. I've fixed it, by deleting & re-creating the two 'loose' segments. I 
still don't understand what happened though.

Having sent the fix before I thought to say, it won't now be recreatable, 
sorry.

This was a fresh download, not a saved file. Merge nodes didn't work, because 
I couldn't select the non-existent node...
