Besides those junctions, I think that Madrid is one of the few cities with
street lights *inside* roundabouts...

Map with topology in mind:
- Roads at different heights = different ways
- Roads with a median[1] of *any* size = different ways.
- Draw segments in the direction of the traffic

And don't worry about the rendering. Let the renderers worry about it. Hacking
the renderers (osmarender and mapnik) can be pretty tricky.
