I've had fits with rivers too and still haven't got it down pat.  One
thing I'd try though is either make the left/right banks one way (one
stretch of the example river had two ways on each bank), or try making
the outer way one closed polygon.

If you nail it down, shoot back a message with some best practices.  :)
