To continue in my role as de facto fact checker for Steve C's posts, here
is the link to the first notice about the membership record discrepancies
(to my knowledge):

Please note this quote: "As we became aware from the last election, some
members who are eligible to vote did not receive a ballot due to a bug in
our membership management software and PayPal payments."
Unlike Steve, I don't think anyone tried to "pretend it was totally fine"
However, it is a problem that was identified after the last election, as
having been a factor in the last election.
