 I've just been trying to reproduce this on the test server and it is quite
 random but very reproducible; I can get it to happen 10-20% of the time.

 In this changeset, I drew a way around the visible edit area, then started
 dropping in confectionery stores. Some distance to the north you can see
 the ones that were added incorrectly. While these were all added to the
 north, an earlier experiment dropping in phonebox pois as part of the same
 changeset produced offscreen pois with west, south west, and south
 offsets.

 Playing with the editor it seems the 'wrong' offset is something like
 double the distance from where the editor was originally centred - ie load
 potlatch, move slightly west, and when a point disappears it will be even
 further west. If you drag the edit area a large distance north, the
 misplaced poi will be a yet larger distance north. I've not measured to
 see if it's actually doubled but the effect of recentreing the map is 100%
 reproducible.
