I think I'd be inclined to change it so that when there is no image we show the normal list but without the "keep" and "remove" options. Either don't highlight any by default, or add a "No image" option at the top which is selected.

As far as size goes, we actually keep the original image, and two resized versions. The URL in the user API response for example is the original, at whatever size the user gave us. There may be an argument for doing away with the resized versions and just using the original with client side scaling - in particular I guess it may give an advantage on retina displays? We are already doing that for the latest even smaller size images in some of the work @tmcw has done.

As to only using gravatar, well that is probably what I would do if I was starting from scratch now, and there is certainly an argument for defaulting to it for new users. The question is how much will be upset people if we just dump all the local images...
