I think I agree with @tordans that dropping the colons would be better - it's also consistent with the forms on the settings and preferences pages for example.

He means having the label line with `Password` left aligned and `Lost your password?` right aligned - possibly that means making it two columns? Then the text entry is underneath. It sounds reasonable to me.
