[Routing] Complex Access Tag Interaction - Wiki vs. Data

What's your take on access tags and parsing complex access tag hierarchies?

The most recent example we stumbled upon was

> highway=steps + access=permissive

for which the Wiki states that the access tag applies to _all_
transportation modes. This would mean cars should route over steps, which
makes no real sense.

I'm quite interested in ideas what to do in cases where the Wiki states one
fact but the data out there is mapped in a different / the opposite way.
