I'd say a standalone toilet should stick to wheelchair=yes  to keep with
the well established wheelmap.
But a place might have:

shop=bakery
toilets=yes
toilets:wheelchair=limited
toilets:supervised=yes

But for any proposal involving accessibility, could you get someone from
