in a retail store by purchasing a "top-up" or "refill" card at retail.
These cards are stamped with a unique code (often under a scratch-off
panel) which must be entered into the phone in order to add the credit
onto the balance.

How would you tag this?

mobile_top-up_card or mobile_top-up_voucher would be the first thing
that comes to my mind if we want BE, at least that what I found at UK
providers.

Although something like mobile_refill_card or mobile_prepaid_card (or
then rather voucher so there is no confusion with a prepaid sim card)

Oh and is vendin=telephone_vouchers a good tag for a
https://en.wikipedia.org/wiki/Telephone_card? Looks like nobody calls
them like that. Btw. is it really telephone card or phonecard and not
phone card? We probably have to doulecheck that anyway, because I bet
many are for mobiles as there was no documentation.
