in Germany and Italy (and probably some more places) the difference is between a road section without grade level intersections (and with ramps) vs not. Trunk is used in these areas for roads that are built to a standard similar to a motorway but not legally designated as motorway. It is not about access restrictions (there is the orthogonal motorroad=yes property for this).
