I'm not reproducing this issue in iD v2.20.0: Handrail and Tactile 
Paving add handrail=no and tactile_paving=no, respectively, when you 
click on each one twice to change it from Unknown to Yes to No. iD's 
preset system does support a two-way checkbox field that adds no tag 
when unchecked, but as far as I can tell, it isn't being used for 
anything at the moment.

If you're able to reproduce this issue reliably, please open an issue in 
iD's bug tracker with more details to reproduce this issue. Thanks!
