[Tagging] Areas around doctors/dentists - or any other amenity

It depends

For urban dentists, say where it's one of a building terrace, then I'd 
add the tags of the building, or if it's on upper floors of a building 
with, say offices/shops below I'd add it as a node.

For more expansive one, with gardens/grounds/parking etc I'd tag the 
site's boundary, similar to most schools.

That's quite generic. If there's more detailed data available, it's 
better to use it.
