Ok, here goes:

In 50 words or less, describe the event/organization you represent:

OpenStreetMap is an organization aimed at mapping the world with
user-collected data. Contributors collect data using GPS and share it with
others to create an overall map of the world. Mapping parties are gatherings
to attract local people to help gather map data using handheld GPS devices.

If products are requested, please list the model and quantity:

Handheld GPS devices with a screen displaying current location to ease with
note taking, high accuracy/low drift, and the ability to save a recorded
paths for later download to computer. The Garmin 60Cx seems to be the
closest with the features we need. The mapping parties tend to have 10-20
people attend -- ideally we'd like one per person.

What is required from Garmin:

GPS product donation and some information or pamphlets to give out about
your sponsorship at our mapping parties.

What is the proposed term of the agreement:

(I'm not sure what this is referring to. Is term in this case the length of
the agreement?)
