These are all good questions!

Our wiki says "longitudinally placed rumble strips to alert drivers that they are leaving their lane...are generally not mapped by OSM." [1]

shoulder:width=4 (for four meters, for example)

Not really, although there is a proposal (inactive since 2007!) here [2] for additional structures on bridges, which might be better extended to include such "lateral protection." Take a look at the Golden Gate Bridge sidewalks (East and West, [3]) where some rather detailed tagging is included, although none seems to indicate the "suicide prevention measures" (my choice of words) that the Bridge District has taken to make the bridge safer for pedestrians and cyclists.

These are absolutely more helpful. Tags like shoulder=no, surface=* and smoothness=* are especially welcome additional tags quite useful to cyclists. The recently-Approved hazard=* key can also be used to indicate areas (as nodes or ways) where specificity might even cause a cyclist or router to avoid an area or re-route. Although, on USBRs, such hazards are virtually or actually unknown, as USBRs are designed to be "safer and more comfortable" for cyclists (while recommending that they be utilized by adult riders with good, confident cycling skills on public roads sometimes shared with automobile traffic).

BTW, the list of USBRs yet-to-enter for the Spring 2021 AASHTO Round has shrunk in the last few days: in addition to USBR 66 in California needing completion (thanks again, Volker, for whatever you might add!) remaining are six routes in Ohio, the single route in Indiana, and USBRs 20 and 81 in Washington state (both are seeded); Utah is complete — big thanks to user:Aweech! We're about halfway done entering these, keep up the great work, everybody who does so!
