Ok, no problem here. Just skip the city part off the highway. Way's can
contain holes - no problem here.
Example:
A -------ab------- B -----bc------ C ------cd------- D
====================---------------===================
A, B, C, D are nodes
ab, bc, cd are line segments
===== ====== is the highway. It contain a space, since the highway
isn't a highway within the city (as you suggest)
---- is the city road.
