As the one putting most of Copenhagen on the map, I can tell you that 
everything there is handdrawn on top of my gpx tracks. My procedure is 
as follows : Load gpx file in JOSM. Download OSM data for area covered 
in gpx file, plot new nodes based on the gpx file, add segments between 
nodes, make ways from segments and tag them appropriately. Upload data 
to OSM, gzip and upload processed gpx track to OSM.

If I had used the JOSM gpx conversion to datalayer, and just uploaded 
that, I'd be in the top of the daily/weekly/monthly stats, since there 
would be a node and a segment for each second of my track.
Personally I'd say that automatic generation of segments are for the 
lazy, and will never be able to do as good a job as a human. And if 
there were that feature, the lazy will just use it, and not check and 
correct the data further.
