The best option.

2) modify the downloaded file myself to store the bounding box that I

They are rendered afterwards and so should not have any effect.  There is a
setting to turn them off.

- Do line-widths and end-caps have any effect when there's a wide road at

Since the edge is clipped this should be OK, but Osmarender isn't doing the
clipping properly yet (it makes a white border which you normally cannot
see).

It would do if the clipping was done properly.  Don't really know if it will
be any better or different.
