There is a nice (and small) patch* floating around that creates a node and also a
segment back to the previous node. I think Imi has it in mind to add to JOSM
sometime soon from what he said on IRC. Unfortunately it no longer applied to
the latest version when I tried.

Neither are yet possible, unless things have changed very recently.

* Sorry, can't remember the name of the person who created it just now, but it
is in the dev archives for about a month ago.
