That is true. However, if you choose a really big bounding box, like the whole 
of the world, the database will take at least 45 minutes of hard I/O to sort 
through, and determine which points are the first unique 5000 according to the 
query.

I presume someone was opening Josm then with a new map, trying to download GPX 
points for the whole map repeatedly.
