For that I use Areas with alternating orientation (clockwise and
counterclockiwse) that share the bordering segment on both ways.
Renders in Mapnik and Osmarender (Tiles at Home), can create problems for
merges, but these can be overcome with JOSM's "reverse segments"
feature. Also there is still an issue with Mapnik showing the borders
between the areas, but you cannot make one large area for the entire
river, because you would never be able to sensibly download and edit
such a beast.

I agree, for that you might want to add a "river" along the center line
of the natural=water area, or something similar.
