If the off-ramp or spur isn't yet built, it won't be in the Sat-nav's
map, so the unit will never guide someone off it, unless there is a
servere GPS/INS or software screw up.

Yes, and I don't know of any commercially available navigation systems
that consider elevation in these terms. Most road networks, including
OSM's do not record elevation of nodes or segments.
