Elevated Highways:

For rendering purposes an up-ramp should comprise of two ways.  The first
should have layer=0 and be connected to the lower road.   The second layer=1
and be connect to the upper deck.  The join in the middle will be rendered
seamlessly providing the two parts are not at an acute angle.

I don't know whether this is sufficient for navigation purposes.
